Side 33 - A physician who knows or has reason to believe that a person whom he treats or prescribes for is infected with either gonorrhea or syphilis shall immediately report the name, address, age and sex of such person to the secretary of the state board of health, for which report he shall receive the sum of twenty-five cents to be paid by the state board of health. A physician who fails to make such report shall be fined not more than two hundred dollars.
