alphabetically, and showing for each beneficiary dates of admission and discharge (or remaining), and number of days treated in hospital during the month, together with certified statement furnished by the finance officer as prescribed below:

(1) The finance officer who settles the accounts of the hospital will furnish the officer in charge of the hospital a certified statement showing disbursements made from the appropriations "Medical and Hospital Department" and "General Appropriations, Quartermaster Corps" (exclusive of amounts disbursed on War Department Form No. 351, covering subsistence charges), setting forth the amounts actually disbursed during the month, for both supplies and services, on behalf of the hospital from these two appropriations, arranged by proper project, procurement, and item number designation.

(2) The Surgeon General will, upon receipt of the data outlined in subparagraphs a and b (1), prepare a statement in duplicate covering the proper prorating of expenses of hospitals showing amounts properly chargeable against the Bureau of War Risk Insurance appropriation, and will submit the original of this statement, through the Quartermaster General of the Army, to the Chief of Finance, requesting immediate adjustment between Army appropriations involved and the Bureau of War Risk Insurance appropriation. The duplicate statement will be forwarded to the Director of Bureau of War Risk Insurance, Treasury Department, Washington, D. C. A separate distinctive entry will be made on this statement by the Surgeon General showing the various amounts disbursed during the month from the Bureau of War Risk Insurance appropriation to cover subsistence charges paid on War Department Form No. 351.
