Formaldehyde Gas Generator, No 2.

DIRECTIONS FOR USE.

1452. The capacity of this size Generator is for the Disinfection of rooms or apartments aggregating 30,000 cubic feet of space. Full charge, 128 ounces Formaldehyde and Chloride Calcium in solution (see Formula), but this should be proportionately governed by the number of cubic feet of space to be disinfected, evaporating until about half-an-inch of the solution is visible in the gauge glass.

1453. To charge the Generator, unloosen and throw off the swing

ing bolts, using wrench provided with each apparatus, and remove the dome head. See that the Generator is clean and dry internally, then pour in the compound. Return dome head to place and tighten bolts firmly, so that no emission of gas will occur. Light the oil burner as directed on special card attached. Pass the hose-nozzle through a keyhole in door, or other small aperture, so that the gas may be discharged into the contaminated apartment. When a pressure of 45 lbs. is indicated on the dial of the pressure gauge, open the throttling valve slowly until its limit is reached, then adjust the petroleum burner to maintain this pressure during the period of evaporation. Two sections of hose and two nozzles are furnished in order that two adjoining rooms may be disinfected at the same time, as occasion may require and conditions admit.

NOTE.

1454. 1455. The patient having been removed from the contaminated apartment, no preparation is necessary in anticipation of the work of disinfection, beyond closing tightly the doors, windows and ventilators. Leave all items of furniture, clothing, books, pictures and carpets in the room; the gas will penetrate every object and destroy all bacteria without injury to the articles. After discharging the required amount of gas into the rooms, leave them undisturbed for several hours (say overnight, if convenient), then open the windows and doors, and the gas will quickly disseminate into the atmosphere. The safety-valve is set to relieve, automatically, at about 60 lbs. pressure. We advise that clean water be occasionally placed in the Generator, boiling it, and permitting the steam to pass freely through the valves, rubber tube and nozzle, for cleansing purposes. 1456. CHAPTER VI.

LAWS AND REGULATIONS CONCERNING

VITAL STATISTICS.

N. B. The Regulations of the United States Treasury and the Acts of the State of Louisiana are obligatory. The Regulations of the State Board marked by a star are obligatory also. The other Regulations are recommendatory and educational. Each Local Board will adopt those which it thinks best suited for its local conditions.

1457. Extracts From Act No. 80, 1877, Relative to Registration of Births, Marriages and Deaths, etc.

Section 9. Be it further enacted, etc., That

1458. It shall be the duty of the attending physician or midwife, at the birth of any child in the Parish of Orleans, in the State of Louisiana, to report the same to the office of the Board of Health within twenty-four hours of the same, and

1459. It shall be the duty of the father, or, in his default, of any

person present at the birth of any child, to report within twentyfour hours from the date of said birth, at the office of the Board of Health, such birth, which report shall be recorded in presence of two witnesses, and shall, as nearly as possible, show the date, hour, street and number thereof of such birth, the sex of the child, and its name, the names of the father and mother, their nationality, age, occupation and residence, and the name of said witness.

Section 10. Be it further enacted, etc., That

1460. Every death in the parish of Orleans, or State of Louisiana shall be reported at said office within twenty-four hours after it has occurred.

1461. Such report shall be made by the nearest relative, or the husband or wife of the deceased, if present in the parish.

1462. Otherwise by the executor, if designated, or landlord of the deceased, or the officiating undertaker.

1463. It shall be the duty of coroners to report all deaths coming to their official notice, and

1464. Of the president, manager or superintendent of any State, parish, municipal, charitable or benevolent institutions; 1465. Said report shall, as far as possible, contain the name, sex, age, occupation, residence, color or race, and nativity of the deceased, the cause of death, the street and number where it occurred; shall show whether deceased was married or unmarried, the name of surviving spouse, if any, and where residing; also the name, age, and residence of the declarant, and his relationship, if any, to the deceased, and such other particulars as may be required by said board.

1466. No body shall be removed from said parish until a permit shall have been obtained from the president of said board, under his official seal, showing that the requirements of this section have been complied with.

Section 11. Be it further enacted, etc., That

1467. Every marriage celebrated in the parish of Orleans shall be recorded in said office.

1468. Such record shall show the full names of the contracting par

ties, their age, nativity, date of license of marriage and by whom issued, the names of the parents or tutors of the contracting parties, the name of the officer, priest or ecclesiastic celebrating the marriage, with the date of its celebration and the names of the witnesses thereto, and any such facts as the board may judge necessary for vital statistics.

1469. It shall be the duty of the officer, priest or ecclesiastic cele

brating any marriage to return the license authorizing such marriage to said officer after having endorsed on such license the date of such celebration.

1470. These licenses so returned shall be preserved among the papers of said board.

Section 12. Be it further enacted, etc., That 1471. It shall be the duty of the president of said board to cause the registry of any marriage celebrated prior to the passage of this act, on production of a certificate, duly attested, of the cele bration of such marriage, and in case of the loss of such original certificate, such registry shall be made on the production of the affidavits of the contracting parties and at least one of the

witnesses present, or of the officer, priest or ecclesiastic celebrating such marriage, and a certified copy of such registry shall be prima facie evidence of such marriage.

Section 13. Be it further enacted, etc., That

1472. There shall be collected for the recording, under this act, of any birth or death, the sum of fifty cents, and like fee for any certificate of such record, and for recording any marriage the sum of one dollar, and like fee for certificate of the recording of the same.

1473. The foregoing provisions are hereby adopted and made applicable to all local Boards of Health (under Sec. 7 of Act 192 of 1898, last clause, first paragraph), and physicians, midwives, householders, etc., will report to the local boards, within such time or delay as may be determined by ordinance of the local Boards, which shall fix the period of time in which the reports are to be made according to the facilities for communication in that particular Parish or Municipality, and such report shall be under form 1, and said local boards will report monthly the vital statistics of the preceding month, to the State Board on form 2; said monthly reports to be made at the beginning of each month.

1474. It shall be the duty of the next of kin of any person deceased, and of each person being with such deceased person at his or her death, and of the person occupying or living in any house or premises in or on which any person may die, and of the parents of any child born in said district (and if there be no parent alive that has made such report, ther of the next of kin of such child born), and of every person present at such birth, within five days after such birth or death, to report to said board, in writing, so far as known, the date, ward, and street number of said birth, and the sex and color of such child born, and the names of the parents, and the age, color, nativity, last occupation, and cause of death of such deceased person, and the ward and street and place of such person's death and last residence.

1475. No person shall make, prepare, deliver or issue any false certificate, statement or report of a birth, marriage or death, or any such certificate, statement or report, which is not in accord

ance with the facts of the birth, marriage or death; all certificates, statements and reports of births, marriages or deaths shall be signed by the person purporting to make the same," and no person shall sign or forge the name of another to any such certificate, statement or report.
